Midwest Forms & Supply Partners with Tim O’Brien Homes for the St. Jude Dream Home

At Midwest Forms & Supply, we believe that a strong community is built on a solid foundation. This year, we are putting that philosophy into action in the most meaningful way possible.

We are proud to announce our partnership with Tim O’Brien Homes for the 2025 St. Jude Dream Home® Giveaway in the Milwaukee area. To support this life-saving mission, Midwest Forms & Supply is donating 100% of the materials needed for the home’s foundation.

A Mission That Hits Home

St. Jude Children’s Research Hospital is leading the way the world understands, treats, and defeats childhood cancer and other life-threatening diseases. One of the most incredible aspects of St. Jude is their commitment to the families: treatments invented at St. Jude have helped push the overall childhood cancer survival rate from 20% to more than 80% since it opened more than 50 years ago. Perhaps most importantly, families never receive a bill from St. Jude for treatment, travel, housing, or food—because all a family should worry about is helping their child live.

Groundwork for a Cure

When Tim O’Brien Homes—a builder known for quality, energy efficiency, and community heart—approached us about the Milwaukee Dream Home, we knew we had to step up.

A house is only as strong as the ground it stands on. By providing the concrete forms, ties, and essential supplies for the foundation, Midwest Forms & Supply is literally laying the groundwork for this project. Our donation ensures that more of the proceeds from ticket sales go directly to the research and treatment happening at St. Jude.

The Milwaukee St. Jude Dream Home is more than just a house; it is a symbol of hope. Built by the expert team at Tim O’Brien Homes, the house will feature top-tier craftsmanship and innovative design. Once completed, a limited number of tickets will be sold to the public, with one lucky winner walking away with a brand-new home.

Every ticket sold helps St. Jude kids and ensures that the lab work and clinical trials continue until no child dies from cancer.
